What Is Long-Term Care?
Long-term care is ongoing nursing and personal care for people who can no longer live safely on their own, usually in a nursing home. Many nursing homes also run a skilled nursing side for short-stay rehab patients leaving the hospital, so the terms nursing home, long-term care facility and skilled nursing facility often describe the same building.
What Is Long-Term Care Staffing?
Long-term care staffing is how a building fills open tours with RNs, LPNs, LVNs and CNAs when its own schedule falls short, so residents get the care their plans call for on days, evenings and nights. What each professional may do varies by state and by building.
What Long-Term Care Staffing Covers
Post-acute residents arrive from the hospital needing skilled nursing and therapy from their first day, often at short notice. Book the extra tour as soon as the transfer is set, and the unit isn’t stretched to absorb it.
Vacation weeks, public holidays and planned leave are gaps you can see coming well ahead. Booking per diem CNAs and nurses for those dates lets your regular team take time off without the schedule running short while they’re away.
During a flu or COVID outbreak, staff call out sick at the same time some residents need to be kept apart with a team of their own. Post the extra tours once the plan is set, and nearby professionals can pick them up.
Who Covers What in a Nursing Home
Most nursing home shifts are covered by one of six roles. What each role may do varies by state and by building.
| Role | What they cover in a nursing home |
|---|---|
| CNA | Bathing, dressing, meals and resident rounds on the hall |
| LPN or LVN | Medications, treatments and wound care, within state rules |
| RN | Assessments, care plans and nursing supervision |
| PT or PTA | Mobility and strength therapy for rehab and long-stay residents |
| SLP | Swallowing, diet texture and communication therapy |
| RT or RTA | Ventilator, trach and breathing care on vent units |
Nursing Home Staffing by Building Type
Long-term care covers buildings with very different residents and very different staffing plans. A skilled nursing wing that turns over rehab patients every few weeks needs a different mix on each tour from a long-stay home where residents may live for years, or from a vent unit for residents on breathing support.
Residents who live in the building for years notice who walks onto their hall. When most tours are picked up by people the building has worked with before, residents and their families keep seeing faces they recognize.
Subacute units take higher-acuity patients with a heavy therapy load, from complex wound care to daily physical therapy and speech sessions. Cover nursing and therapy gaps together, so rehab sessions keep running while your own team is out. See Physical Therapist Staffing and SLP Staffing
Vent and trach units look after residents on long-term breathing support, usually with nursing and respiratory therapy on each tour. When an RT or nurse calls out, post the tour straight away and it goes to licensed staff nearby. See Respiratory Therapist and RTA Staffing
What Our Expert Says
Nursing home staffing is as much about your own team as the open tour. Every double shift and mandatory overtime wears down the people you most want to keep, so using per diem cover for the gaps protects your regulars’ days off and helps keep them in the building.

Fast Answers To
Frequently Asked Questions
You post a shift and it goes straight to CNAs and nurses near you. 95% of shifts are covered when posted a day in advance, and you can post a same-day shift when a tour comes up short before it starts.
Often it’s the same building. A skilled nursing facility is the Medicare-certified side that takes short-stay rehab patients, while long-term care covers residents who live there. Many nursing homes run both under one roof and one plan.
You pay an hourly rate for the role you booked, and it costs nothing to post a shift. Rates reflect local pay, so they differ by area, and there’s no minimum commitment for the tours you post.
We connect you with a replacement at no extra cost as soon as we know. If a CNA or nurse cancels ahead of the shift, a replacement is looked for automatically. If the shift can’t be filled, you’re not charged for it.
Credentials are checked before a CNA, LPN, LVN or RN can take a tour. None of that lands on your desk, so the time you’d otherwise spend vetting goes back to your residents and the floor.
Yes. Post holiday and vacation tours as soon as the dates are set, weeks ahead if you like, and per diem CNAs and nurses close by will see them. Planned gaps posted early give more people time to pick them up before the date.
Yes. Skilled nursing rehab books physical therapists, PTAs and SLPs to keep therapy sessions running while your own therapist is out. See Physical Therapist Staffing and SLP Staffing for how therapy shifts work on Clipboard.
Yes. You can post one tour, day, evening or night, for an LPN, LVN or RN, with no minimum number of shifts and nothing to sign. Post a run of tours when you’re covering a vacancy, leave or a holiday week.
Yes. If a CNA or nurse is a good fit for your building, you can offer them a permanent role. There’s no placement fee and no conversion fee for making the offer, and the professional decides whether to accept.
